Some of these are pre-filled. Ensure accuracy wherever you need to write.
2. Name (In Capital Letters)
Write your name exactly as per your matriculation certificate.
Avoid:

- Nicknames
- Spelling variations
3. Father’s Name
Write full name clearly.
Be prepared to answer questions about:
- Occupation
- Nature of work
- Daily routine
Residence Details (Very Important Section)
You must fill three different entries:
- Place of Maximum Residence
- Present Residence
- Permanent Residence

Educational Record
You need to fill details from:
- 10th (Matriculation)
- 12th
- Graduation
- Post-graduation/Professional (if applicable)
Include:
- Institution name
- Board/University
- Year
- Marks/Division
- Medium of instruction
- Boarder/Day scholar
- Achievements
Assessors observe:
- Consistency
- Academic discipline
- Sudden improvement or decline
Physical Details And Current Status
Includes:
- Age (Years + Months)
- Height (in metres)
- Weight (in kilograms)
- Present occupation
- Monthly income (if any)
Be accurate. Do not manipulate these figures.
NCC Training
If applicable, mention:
- Wing (Army/Navy/Air)
- Division
- Total training
- Certificate (A/B/C)
If you don’t have NCC, simply write No.
Participation In Games And Sports
Mention clearly:
- Name of sport
- Duration of participation
- Level (School/College/University)

Nature Of Commission And Choice Of Service
You must specify:
- Type of commission (Permanent / Short Service)
- Preference (Army/Navy/Air Force)
Be clear and logical. You may be asked to justify your choice.
Number Of Attempts And Previous SSB Details
You must mention:
- Total number of attempts
- Details of previous SSBs (if any)
Including:
- Entry type
- SSB centre
- Date
- Chest number
Never hide previous attempts.
